Ontada Health offers secure, online access to your health records. Ontada Health allows you more meaningful engagement with your care team — streamline communication, request appointments, and access treatment history, including diagnosis information, medications, lab results, and more.

Registering for online access to your medical information is easy at the clinic. If you are interested in signing up, talk to your front desk staff about registering for Ontada Health. Once a clinic staff member sets you up, you will receive an enrollment invitation by email, usually within 24 hours. Once you activate your account, you will be able to sign in to Ontada HealthSign via your computer, phone, or tablet.

What is Ontada Health and its features?

Ontada Health is your link to your health information from your physician. Your health record is always
available when you want to see upcoming appointments, lab results, and medications, send a message
to your care team, and more. Ontada Health is provided at no cost to you.

With Ontada Health, you can:

  • Securely message your care team.
  • Receive secure, personalized links to your health information.
  • Request appointments and reschedule appointments.
  • Receive notifications via computer, text, or tablet.
  • Access to health records 24/7 via the web from a desktop, tablet, or mobile device.
  • Invite your caregivers to Ontada Health to access your information and independently support your care journey.

Ontada Health Enrollment

Let your physician’s office know that you want to enroll in Ontada Health. Provide them with your email address to receive an enrollment invitation by email within 24 hours.

Please note you will have 30 days to access the link once you receive the email. The email will provide a temporary password to use to access your Patient Portal page. You will then be prompted to create your own account and personal password.

Tip: Add noreply@ontadahealth.com to your safe sender list. If your invitation does not arrive within 24 hours, first check your junk and spam folders for the invitation. If the invitation is not in your junk or spam mail folder, contact Ontada Health Support at 1-855-887-6788 (toll-free).

Ontada Health Patient Portal Login

To access the Ontada Health Patient Portal, you will need to complete enrollment and create an account. Your practice can enroll you in the Ontada Health Patient Portal. After completing enrollment, sign into Ontada Health.

  • To sign in to Ondata health portal, go to the Ontada Health Login page
  • Enter your email address and password. If you use a computer that only uses access and want to stay signed in to Ondata Health, check the Keep me signed in box.
  • If you sign in using a public computer, make sure to sign out before you leave the computer.
  • To protect your health information, your account automatically signs out after 15 minutes of inactivity.

After logging in, the first thing you see is the For Me page. This is your dashboard where you find info about: your Next Appointment and to request a NEW APPOINTMENT, your To Do List with outstanding registration forms, assessments, and surveys to read and complete, your recent lab Results, your unread Messages, and your Care Team info.

In the top-right:

1. To increase the size of text, click Text Size.
2. To view your account settings, click MY ACCOUNT.
3. To get help with Ontada Health (this guide), click HELP.
4. To sign out of the Ontada Health login portal, click LOGOUT.

Forgot Your Password?

However, if you forgot your password, click forgot password below the verify button and enter your email address to receive an email to reset your password.

Your password must contain at least eight characters, 1 uppercase character, 1 lowercase character, and one number. It may contain special characters. Your password may not contain your first name, last name, username, or medical record number.

If the password reset email does not arrive within 24 hours, check your junk and spam folders for invitations. If the invitation is not in your junk or spam folders, contact Ondata health support at 1-855-887-6788 Toll-free.
